NSW Fertility Rebates

The NSW Government has committed $80 million to increase access to affordable fertility treatment for NSW residents.

If you’re a NSW resident undergoing fertility treatment, here are the rebates you may be eligible for.

Pre-IVF Fertility Testing Rebate

If you are undergoing pre-IVF fertility testing, you may be eligible to receive a rebate. From 1 January 2023, you can receive up to $250 (previously $500) to cover your out-of-pocket expenses for pre-IVF fertility testing.

Eligibility criteria:

Female applicant must be a NSW resident. Note: Eligible fertility tests may be for a man or a woman, but only women can submit a claim for the rebate.

  • You must have been referred for eligible fertility tests by a general practitioner or a specialist.
  • You must have incurred out-of-pocket costs for one or more of the following tests for the purposes of pre-IVF fertility testing:
      - Anti-Mullerian Hormone (AMH) test
      - Pelvic ultrasound
      - Ovulation test
      - Semen analysis.
  • You must have undergone pre-IVF fertility testing from 1 October 2019.

How to make a claim:

  • Keep hold of your receipts for pre-IVF fertility testing.
  • Ask your fertility specialist to complete the Pre-IVF Fertility Testing Rebate Form.
  • Visit https://www.service.nsw.gov.au/transaction/apply-pre-ivf-fertility-testing-rebate and log in (or create) your MyServiceNSW Account.
  • Follow the prompts to apply for a rebate.
  • Once your rebate has been reviewed and approved, you’ll receive your rebate in your nominated bank account within 28 days. This rebate can only be claimed once.

Fertility Treatment Rebate

Patients undergoing fertility treatment may be eligible to receive up to $2,000 (depending on total out-of-pocket expenses). If you have undergone fertility treatment from 1 October 2022, you will be able to submit a claim via Service NSW.

Eligibility criteria:

  • Female applicant must be a NSW resident.
  • You must have undergone IVF, artificial insemination, or another fertility treatment as defined by the ART Act 2007.
  • You must be undertaking fertility treatment in an accredited private fertility clinic (like Monash IVF).
  • You must have undergone fertility treatment from 1 October 2022.
  • You must have incurred out-of-pocket costs during your fertility treatment.
  • You are not undergoing fertility treatment for the sole purpose of fertility preservation.

How to make a claim:

  • Claims are submitted via Service NSW. You can find more information here.
  • You will need to have a completed ‘fertility treatment rebate declaration form’. The declaration form can be found here.
  • The Monash IVF billing team can complete Part 1 of the ‘declaration form’. Please contact them via email at NSWAccounts@monashivf.com.
  • You will need to have proof of your out-of-pocket expenses which can be found by downloading your Medicare benefit statement. To obtain a Medicare benefit statement, you will need access Medicare online. Medicare can be accessed through:
  • When you have your Medicare benefit statement, your completed declaration form, and the required amount of identification, you can submit your claim via the Service NSW website. Visit: https://www.service.nsw.gov.au/transaction/apply-for-the-fertility-treatment-rebate and log in (or create) your MyServiceNSW Account.
  • Follow the prompts to apply for a rebate.
  • Please note: You can only apply once for this rebate.
