The Monash IVF Donor and Surrogacy Team maintain a register of patients who are interested in accessing IUI quality sperm for use in treatment with Monash IVF Victoria.
This register is regularly reviewed to ensure it remains current and enables our patients to access IUI donor sperm as quickly as possible.
Please fill out the below form to confirm your intention to remain on this register, or your desire to be removed from this register.
Please note, your access to the Sperm Donor Portal will only be activated once you have completed your counselling session on the use of clinic recruited donors.
