We are very grateful for the opportunity to share more information about our donor program with you and we have attached an information sheet which covers the requirements for becoming an egg donor, as well as the steps involved. You can also access our Egg Donor Companion to learn more about what it means to be an egg donor with Monash IVF or Repromed. If you are interested in proceeding with the next step, we ask that you complete the attached Medical and Genetic Questionnaire Form so we can review your suitability for donation. You can email this completed form back to us at donate@monashivf.com.


When completing the Medical and Genetic Questionnaire Form, please ensure you include as much detail as possible. This includes any information relating to the medical and genetic history of:
- Yourself
- Your parents
- Siblings (if any)
- Grandparents
- Extended family (blood relatives)
If you are aware of any genetic conditions in your family, please provide as much detail and supporting information as you can, including test results where available. This information will be helpful for our team who review all potential donors.
What happens next?
- Medical and Genetic Questionnaire Form returned
- An appointment with one of our Donor Doctors or Specialists
- Completion of initial screening tests
- Counselling for you and your partner (if applicable)
- Consent and donor profile forms completed
Most appointments will be completed over Zoom, but please advise us if you would prefer an in-person meeting.
